Job Summary


The Mental Health Case Management and Resource Coordinator is responsible for coordinating and enhancing mental health case management services, building and maintaining a comprehensive referral network, and supporting the availability of community and campus resources for students. This role involves resource development, supporting access to low-cost mental health and medical care, and facilitating student referrals to appropriate services. Additionally, the coordinator will deliver direct case management support to students, supervise clinical case management staff, and contribute to developing and delivering training programs for CAPS staff. The position requires strong collaboration with campus and community partners to ensure effective, seamless student care.

What Your Key Responsibilities Will Be


Resource Development
  • Collaborate with community resources to build and maintain a strong referral network and streamline the referral process.
  • Partner with CU campus departments and resources to enhance knowledge of campus resources and simplify referral access.
  • Be responsible for and update the Screen Guide and referral resource document for CAPS staff to ensure accurate, efficient referrals to the community.
  • Enhance CAPS referral resources, focusing on low-cost mental health and medical care, basic needs, and financial support options.
  • Work with SHIP and Gold Insurance providers to gather information on annual insurance changes, including in-network and out-of-network coverage options and costs.
  • Coordinate with all CAPS teams to ensure proper maintenance and dissemination of resource lists.
  • Collaborate with Thriving Campus to manage and update the referral website, staying current with new features.
  • Provide training for CAPS staff on using the Thriving Campus referral platform.
  • Partner with FSAP and OVA to remove community referrals when concerns arise.
  • Recruit and support new community referral partners, assisting with the addition of profiles to the Thriving Campus platform.
  • Work with the Crisis & Triage Team to facilitate referrals to crisis services when client acuity increases.
  • Coordinate with Psychiatry to support community referrals and access to low-cost medications.
  • Manage Teams chats for referral requests.
  • Be responsible for the CAPS Food Pantry and snack availability for students.
  • Collaborate with the CAPS Director and AD of Clinical Operations to determine optimal use of available funds for student care access.

Direct Mental Health Case Management
  • Meet with students in person and via telehealth to provide individual case management services.
  • Document all student interactions in the Medicat Electronic Health Record (EHR).
  • Support students in accessing care through phone calls, secure messaging, and text communication.
  • Facilitate open, collaborative communication with both campus and community providers to ensure effective client care.

Supervision
  • Supervise bachelor-level clinical case management providers.
  • Work with the AD of Clinical Operations to define and implement clinical case management services within CAPS.
  • Lead the recruitment and training of new case management staff.
  • Provide weekly one-on-one supervision for clinical case managers.
  • Develop schedules and monitor caseloads for clinical case management staff.

The hiring range for this position is $61,200-$76,500 annually.

What We Require

  • Master's degree in Social Work, Psychology, or a related field.
  • 2+ years of experience in case management services.
  • Familiarity with Boulder County and Denver Metro resources.
  • Experience collaborating with community-based resources.
